SMILE, or Small Incision Lenticule Extraction, is a single-step technique where the surgeon uses a laser to directly cut the middle layer of the cornea and creates a small incision measuring 2-4 millimeters. The cut portion of the cornea is then removed through the incision, altering the curvature of the cornea to achieve the desired refractive correction. The procedure eliminates the need to create a corneal flap, resulting in a significantly reduced wound area of up to 80% and improved wound healing. Unlike LASIK, the SMILE procedure involves laser cutting of the cornea without any odor, while LASIK can produce a burning smell during the laser ablation of the cornea. The purpose of Corneal Collagen Cross-Linking (Smile XTRA) surgery is to strengthen the cornea by increasing the number of "anchors" that connect collagen fibers together. This enhances the resilience of the cornea and reduces the damage to the corneal layers during the vision correction process. The doctor will apply an eye solution containing vitamin B2 (riboflavin) to the patient's eyes and then expose the eyes to UVA light for approximately 1 minute. This process allows the collagen proteins within the corneal fibers to bond together, achieving corneal cross-linking and strengthening.
